ABSTRACT

Purpose: The author describes a case of bilateral subretinal and sub-RPE hemorrhage due to age related macular degeneration treated with injections of tPA and perfluoropropane gas. Case report: Two eyes of a patient with subretinal and sub-RPE macular hemorrhage received injections of 25 mug of tPA and 0,3 mL of C3F8. The patient was assessed regarding visual acuity, retinography and fluoresceinography before and after the procedure. Results: One week after treatment, both eyes presented with complete resolution of subfoveal hemorrhage, however traces of subretinal hemorrhage were found in both eyes, mainly in the left eye. Sub-RPE hemorrhage remained the same during the treatment of the left eye. Visual acuity improved in both eyes, specifically in the right eye. Conclusion: Intravitreal injection of tPA and perfluoropropane gas seems to be an alternative procedure in the treatment of subretinal hemorrhage due to age related macular degeneration, however, sub-RPE hemorrhage remains a therapeutic challenge. Further studies are necessary.

RESUMO

Objetivo: O autor descreve um caso de hemorragia sub-retiniana e subepitélio pigmentado da retina (EPR) bilateral, secundária a degeneração macular relacionada à idade, tratada com injeção intravítrea de ativador do plasminogênio tecidual (tPA) e gás perfluoropropano (C3F8). Relato do caso: Dois olhos de um paciente portador de hemorragia sub-retiniana macular e sub-EPR foram tratados com injeções intravítreas de 25 mig de tPA e 0,3 mL de C3F8. Avaliaram-se a acuidade visual e o aspecto da retinografia e angiofluoresceinografia pré e pós-operatória. Resultados: Uma semana após as injeções, os dois olhos apresentaram resolução completa da hemorragia subfoveal, porém resquícios de hemorragia sub-retiniana perimacular foram observados, mais acentuados no olho esquerdo. A hemorragia sub-EPR localizada no olho esquerdo, não apresentou sinais de melhora. A acuidade visual apresentou melhora em ambos os olhos, principalmente no olho direito. Conclusão: A injeção intravítrea de tPA associada ao C3F8 apresenta-se como uma alternativa viável no tratamento da hemorragia sub-retiniana secundária a DMRI, porém hemorragias situadas abaixo do EPR ainda permanecem um desafio terapêutico. Estudos futuros são necessários.
