Abstract

Objetivos: Quando a visão de um olho está preservada (visão monocular) e há alto risco, baixo prognóstico e/ou recursos limitados para a cirurgia do olho contralateral, não é claro se o beneficio da binocularidade supera o da reorientação para visão monocular. O objetivo é quantificar o impacto da qualidade de visão referida entre a condição binocular e monocular e, nesse último caso, entre congênita e adquirida. Métodos: Pacientes com acuidade visual (AV) com AV>0,5 em cada olho foram submetidos ao questionário estruturado de 14 perguntas (VF-14), onde a pontuação de 0 a 100 indica o nível de satisfação do paciente com sua visão, variando de baixo a alto respectivamente. Dados epidemiológicos e pontuações dos quatro grupos foram registrados e submetidos à análise estatística. Resultados: A entrevista pelo VF-14 com 56 indivíduos revelou que a pontuação mais alta foi similar entre os controles e os portadores de visão monocular congênita, e níveis intermediários e baixos foram obtidos por indivíduos com visão monocular adquirida e cegos bilaterais, respectivamente (p<0,001). As atividades mais difíceis para os indivíduos com visão monocular adquirida foram identificar letras pequenas, reconhecer pessoas, distinguir sinais de trânsito e assistir TV. Conclusão: O estudo confirmou que a perda da visão tem impacto desfavorável no desempenho referido das atividades sendo maior na visão monocular adquirida do que na congênita. Os dados sugerem que medidas de reabilitação devem ser consideradas para melhorar a qualidade da visão em doenças intratáveis ou de alto risco ou de baixo prognóstico.

Abstract

Paciente de 65 anos, sexo feminino, foi encaminhada para avaliação de cirurgia de catarata. Relatou história de cirurgias mandibulares para correção de deformação facial desenvolvida ao longo da infância e adolescência. O exame oftalmológico completo mostrou estrias angióides bilaterais, drusas em ambas as áreas dos discos ópticos e membrana neovascular sub-retiniana na mácula esquerda. A análise genética revelou mutação no gene SH3BP2 compatível com o diagnóstico de Querubismo. A avaliação clínica e laboratorial descartou outros distúrbios sistêmicos. O Querubismo é uma doença óssea rara caracterizada pelo desenvolvimento de lesões fibro-ósseas indolores na mandíbula e maxila durante a primeira infância. Os achados oftalmológicos nesta doença estão principalmente relacionados ao envolvimento ósseo orbitário. Este artigo descreve pela primeira vez a ocorrência de estrias angióides e drusas de disco óptico no Querubismo. Enfatizamos que essa condição deve ser incluída no diferencial de pacientes com tais achados, principalmente quando ambos existirem em associação.

Abstract

Unvaccinated identical twins developed bilateral anterior uveitis soon after the onset of coronavirus disease 2019 symptoms. During follow-up, both patients developed choroiditis, and one twine developed posterior scleritis and serous retinal detachment. Prompt treatment with oral prednisone ameliorated the lesions, and no recurrence was observed at the 18-month follow-up. Choroiditis may rarely be associated with severe acute respiratory syndrome coronavirus 2 infection, and it responds well to corticosteroid therapy. Although the exact mechanism is unknown, we hypothesize that the virus may act as an immunological trigger for choroiditis.

PURPOSE: This study aimed to evaluate the total macular thickness as well as the thickness of the inner and outer retinal layers in patients with Parkinson's disease. It also aimed to verify the correlation of these parameters with motor symptoms and cognitive function.
METHODS: A total of 46 eyes of 23 patients with Parkinson's disease and 40 eyes of 20 healthy controls were included in the study. The patients' cognitive, functional, and nonmotor symptoms were evaluated using the Katz Index of Independence and Pfeffer's Activities of Daily Living, Mini-Mental State Examination, Frontal Assessment Battery, Schwab and England Staging Scales, and Movement Disorders Society Nonmotor Symptoms Scale. The macular thickness measurements obtained via total, inner, and outer optical coherence tomography were recorded. Furthermore, the correlation of the parameters of optical coherence tomography with cognitive, functional, and nonmotor symptoms was assessed.
RESULTS: The scores of the Katz Index of Independence and Pfeffer's Activities of Daily Living as well as the Movement Disorders Society Nonmotor Symptoms Scale were significantly lower in patients with Parkinson's disease than in healthy controls. Moreover, the former had greater total macular thickness. The temporal and inferior outer sectors were significantly greater for the ganglion cell complex thickness in patients. A significant correlation was observed between the total macular thickness and the Movement Disorder Society-Unified Parkinson's Disease Rating Scale, Parte III (MDS-UPDRS-III) values. Contrarily, there was a negative correlation between the outer macular thickness and the MDS-UPDRS-III values. Meanwhile, the total macular thickness and ganglion cell complex thickness were significantly correlated with the scores of the Mini-Mental State Examination, Schwab and England Staging Scale, Frontal Assessment Battery, and Katz Index of Independence and Pfeffer's Activities of Daily Living. In addition, the Schwab and England scale was correlated with the outer macular thickness.
CONCLUSION: The total and inner macular thicknesses at the temporal and inferior outer sectors were greater in patients with Parkinson's disease than in the control group. These findings indicate that macular thickness may be greater in those with Parkinson's disease, particularly when associated with mild motor symptoms. In addition, the parameters of the total, inner, and outer optical coherence tomography were significantly associated with motor and nonmotor symptoms as well as cognitive function impairment.
